We are seeking a Primary Care Physician to join a mission-driven interdisciplinary team providing personalized, coordinated care to seniors through a value-based care model.

Position Highlights:

• Full-time, onsite position in Stockton, California

• Monday–Friday, 8:00 AM–5:00 PM

• Approximately 8 patients per day

• No hospital rounds

• Home visits included

• Rotating phone-only on-call coverage a few nights or weekends per month

Responsibilities:

• Conduct comprehensive in-person patient assessments

• Collaborate with the interdisciplinary team to develop individualized care plans

• Coordinate patient discharges and short- and long-term placement needs

• Provide holistic primary care to older adults

Qualifications:

• MD or DO degree

• Completion of an ACGME-accredited residency

• Active, unrestricted California physician license

• DEA registration and applicable California CDS certification

• Board certification preferred

• Background in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics

• Eligible to enroll as a provider for Medicare and Medi-Cal participants

• Must be able to obtain CPR and First Aid certification within 180 days of hire if not already certified
