We are seeking a Primary Care Physician to provide comprehensive, relationship-based care to older adults through a value-based PACE model.

Unlike a traditional high-volume primary care environment, physicians in this practice typically see an average of 8 patients per day, allowing more time to understand each patient's medical, functional, and social needs. You will work closely with an interdisciplinary care team to develop and coordinate individualized care plans for a vulnerable senior population.

Position Overview

The Primary Care Physician will provide comprehensive care to Medicare and Medi-Cal participants through a collaborative, interdisciplinary model.

The position includes clinic-based care and home visits but does not require hospital rounds. Physicians work regular weekday hours with limited rotating phone-based call responsibilities.

This opportunity may be particularly attractive to physicians in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics who want to practice in a lower-volume environment with an emphasis on longitudinal and holistic patient care.

Key Responsibilities

Primary & Geriatric Care

• Perform comprehensive in-person patient assessments
• Diagnose and manage acute and chronic medical conditions
• Develop individualized treatment and care plans
• Provide ongoing primary care for an older adult population
• Conduct home visits as part of the regular work schedule
• Maintain an average patient volume of approximately 8 patients per day

Interdisciplinary Care

• Lead and collaborate with the interdisciplinary care team
• Develop personalized care plans based on each participant's medical and functional needs
• Coordinate services across the patient's continuum of care
• Communicate with clinical and support professionals involved in each participant's care

Care Coordination

• Coordinate discharge following acute care episodes
• Support transitions involving short-term and long-term placements
• Help ensure continuity of care across healthcare settings
• Maintain appropriate clinical documentation and regulatory compliance

Required Qualifications

• MD or DO degree
• Successful completion of an ACGME-accredited residency program
• Active, unrestricted California physician license
• State controlled substance certification as applicable
• Active DEA registration
• Eligible to provide services to Medicare and Medi-Cal participants
• Ability to obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter

Preferred Qualifications

• Board Certification
• Experience providing primary care to older adults
• Geriatric or senior-care experience
• Experience working within interdisciplinary or value-based care models
• CPR and First Aid certification at the time of hire

Schedule & Practice Highlights

• Monday through Friday, 8:00 AM–5:00 PM
• Weekends generally off
• Approximately 8 patients per day
