PRN Case Manager (RN)

Valley View is looking for RN Case Managers with experience in hospital and Epic preferred! Days/Rotating weekends! We're looking for a PRN Case Manager RN to join our Case Management team! We will hire a Registered Nurse or MSW for hospital discharge planners onsite at Valley View Hospital. Case Management and Epic experience a plus! Previous Case Management experience strongly preferred! GENERAL OBJECTIVES: Assists Valley View inpatients and outpatients and their families in the overall Management of their plan of care across the continuum. QUALIFICATIONS: Education: BSN RN preferred. Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program State licensure as a Registered Nurse (RN). 1 year of nursing (RN) experience Experience: Case management experience preferred. Computer experience (word processing, spreadsheet) and familiarity with Meditech We are interested in every qualified candidate who is eligible to work in the United States. However, we are not able to sponsor visas SALARY RANGE: $39.96 to $59.95/hour +14% PRN Differential Pay. Entry salary dependent upon education, skill set, and experience.
