Position Summary As a Registered Nurse Case Manager , you will be at the heart of the interdisciplinary care teamensuring patients receive optimal, efficient, and high-quality care from admission through discharge. This role blends clinical judgment, advocacy, and coordination to improve patient outcomes, support utilization review, and drive seamless transitions of care. ️ Key Responsibilities Conduct comprehensive assessments of patients medical and psychosocial needs Develop, implement, and monitor individualized care management plans Collaborate with the care team to ensure appropriate level of care, resource utilization, and discharge planning Facilitate interdisciplinary team meetings to align care strategies Evaluate medical necessity and level of care using evidence-based criteria Act as liaison between patients, families, providers, and payers Identify potential barriers to care and proactively work to resolve them Maintain compliance with all regulatory, quality, and documentation standards Provide education and support to patients and families, especially for those with chronic or complex conditions Coordinate referrals to community resources, disease management programs, and support services Track trends, analyze data, and make process improvement recommendations Qualifications Active RN license in Florida (or compact license with FL endorsement within 90 days) Associate Degree in Nursing required ; BSN preferred Minimum of 3 years of recent Case Management experience in an acute care setting OR 3 years of experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED Candidates with Home Health or Insurance CM experience must have at least 1 year of acute care experience within the last 5 years Certification in Case Management, Utilization Review, or Nursing preferred Schedule & Compensation Available Shifts: Day Shift: 8:00 AM 4:30 PM EST (1 weekend per 4-week schedule) Evening Shift: 12:00 PM 8:30 PM EST (1 weekend per 4-week schedule) Compensation: Base Range: $31.94 $43.92/hr (based on experience) Shift Differentials: Evening: +$2.50/hr Weekend: +$2.00/hr Differentials are stackable Bonuses & Benefits $10,000 Sign-On Bonus Paid in 5 installments over 2 years Relocation Assistance Available on a case-by-case basis Opportunities for career advancement and continuing education ️ Why Join Us?
