As part of a multidisciplinary service committed to prevention, early intervention, treatment and ongoing management of health conditions, this role offers the opportunity to make a meaningful difference in the lives of people living with complex subacute and chronic health conditions. Community Health and Integrated Care provide a diverse range of community-based services across the ACT, supporting individuals and families through every stage of life. Working within the Community Care Program, Community Nursing Service, you will play a pivotal role in delivering expert clinical leadership and care coordination that supports people to remain living safely and independently in their homes and communities for as long as possible. As the Complex Care CNC, you will provide advanced clinical expertise, support the management of complex patient presentations, and work collaboratively with interprofessional teams to deliver coordinated, high-quality care.
