As a Senior Backend Software Engineer on our Diagnostics team, you'll build the part of Regard that reads a patient's medical chart — labs, medications, vitals, procedures, prior notes, history — and determines which conditions are present, along with the evidence supporting each one. You'll design and develop critical backend services and drive them end-to-end through the full product lifecycle: ideation, planning, design, execution, deployment, and iteration. Our Tech Stack: Backend: Python, Go, PostgreSQL, Redis, Celery Frontend: TypeScript, React Infrastructure & DevOps: AWS, Docker, Kubernetes, GitLab CI/CD, Datadog AI: OpenAI, Anthropic Clinical standards: FHIR, ICD-10, SNOMED, LOINC, RxNorm Responsibilities: Collaborate with product, design, and clinicians to design medium-to-large projects, often starting from an ambiguous problem statement and balancing speed to market, user experience, business goals, and technical architecture quality Own backend services and drive improvements across system design, observability, testing, performance, and support Model clinical data and design the schemas and service contracts that new product surfaces are built on Work directly with clinicians to understand what a diagnosis requires, and turn that intent into logic and tooling they can extend themselves Mentor and review code from other developers across backend services Maintain and improve the quality of the codebase and tooling in your area of responsibility through proactive fixes and by proposing projects that improve the overall backend codebase Participate in operational activities including on-call, monitoring, and production support Minimum Qualifications: BS in Computer Science or equivalent experience 6+ years of professional experience with software development Proficient in Python or Go with demonstrated ability to write and maintain production-quality code Proven experience building data-intensive applications, backed by strong data fluency in designing and evolving schemas consumed across cross-functional teams Able to participate in on-call operational support for your areas of responsibility Able to travel up to 4 weeks a year for company co-working and/or retreat weeks Practical experience leveraging AI coding tools in day-to-day software development (e.g., Cursor, Claude Code, Codex) Preferred Qualifications: Exposure to clinical or healthcare data (e.g., FHIR, EMR integrations, clinical terminologies, or revenue cycle) Comfortable with systems that produce ranked, scored, or probabilistic output and evaluating that output against imperfect ground truth Experience in a startup and/or high-growth environments Experience building tools for non-engineer domain experts Experience managing cloud infrastructure and LLM integrations Experience working on a mature enterprise SaaS product Hybrid Work | Location | Work Authorization For this role, Regard is currently only considering candidates who are authorized to work in the US without visa sponsorship, and are within the New York City metro area, San Francisco Bay Area, or Los Angeles Our expectation for this role is to be in the office 2 days per week. We also require more frequent in-office work during the onboarding period and team onsite weeks up to once per month.
