Position Summary The Access Counselor – Specialized Services plays a key role in coordinating access to care for clients, ensuring timely and effectively solutions while supporting business development efforts. Primarily focused on access coordination, this position will contribute to building relationships with referral sources and expanding service opportunities. This position will demonstrate flexibility in meeting the needs of professional sources, collaborate with internal and external stakeholders to identify growth opportunities, and maintain a strong community present through regular outreach, meetings and events. Qualifications Bachelor’s Degree in Psychology, Social Work, Counseling & Guidance, Rehabilitation Counseling, Social Work, Vocational Counseling, Pastoral Counseling, Family Therapy, or related field, Masters preferred. Illinois Certified Alcoholism and Substance Abuse Counselor (CADC) within 24 months or licensed in Illinois as a LSW of LPC preferred Minimum two years’ behavioral health treatment experience Must qualify as a MHP as defined by the Illinois Department of Mental Health Rule 132. Demonstrated clinical competency in assessments and admissions Adequate written skills to accurately complete required documentation within the time frames prescribed Must be emotionally and medically able to perform essential job responsibilities Belief in the mission and vision of Rosecrance Responsibilities: Work hours prescribed and any additional hours deemed necessary for the responsibilities assigned. Schedule and conduct assessment interviews with potential clients and their significant others. Excellent written and oral communication skills. Ability to quickly establish rapport. Computer proficiency required. Ability to navigate and complete documentation in the electronic healthcare record. Ability to accurately complete required documentation within the prescribed time frames. Knowledge of accepted assessment techniques. Formulate initial diagnostic impression and appropriate recommendations. Provide case management and counseling services to assigned patients. Utilize intervention skills as required to place the patient at the appropriate level of treatment services. Maintain knowledge of community, school and recovery resources to make appropriate interagency referrals as required. Provide networking, linkage and follow-up with referral sources. Ensure that patient service records are maintained consistently with standards, rules and regulations. Work with the treatment team to develop the individual treatment plan and assist the patients with and treatment plan objectives. Document information in patient service records consistent with rules and regulations. Participate in staffing sessions to review patient’s progress and establish future clinical direction. Assume on-call responsibilities as scheduled or when necessary. Serve as a member of the Access Team and participate in all team meetings and activities. Understand and comply with all the principles established by the Rosecrance Corporate Compliance Program and Code of Ethics. Perform all responsibilities in compliance with the mission, vision, values and expectations of Rosecrance. Deliver exceptional customer service consistently to every customer. Serve as a role model for other staff, patients and customers and demonstrate positive guest relations in representing Rosecrance. Assume other related responsibilities as assigned by management. As assigned, performs CLIA-waived laboratory testing and related quality control activities in accordance with CLIA requirements, manufacturer instructions, infection control protocols, and organizational procedures.
