Behavioral Line Support Staff Provider (Beh Line)

The following is a questionnaire and employment/contractor inquiry, and authorization for background inquiry. Please complete it entirely so we can begin your application process. You will only be considered for a possible position when all forms are filled out completely. Completion of this questionnaire and background check does not guarantee, imply, or contract you as a counselor or direct care provider. If you are chosen as a prospective provider, you will be notified by the administration to start the orientation process. Thank you for considering us, Carol and Jason Van Ness (Co-Executive Directors) Responsibilities: Provide 1:1 implementation of the individual’s Behavioral Support Plan under the supervision and oversight of a Behavioral Consultant. Deliver acute, short-term behavioral interventions to address identified challenging behaviors that pose a risk to the individual’s health and safety and/or the safety of others. Maintain focus on immediate stabilization, safety, and risk reduction during behavioral episodes. Support individuals transitioning from institutional settings or at risk of institutional placement. Utilize approved crisis intervention strategies, with Safety Care and/or TCI approaches recommended at the time of enrollment. Provide Behavioral Line Services in the individual’s home and/or in community-based settings, including day programs, by supporting the individual and assisting program staff in maintaining safety and implementing behavioral strategies.
