Melbar is a leading Aged Care Service provider dedicated to providing seniors with high-quality in-home aged care services. Providing various home care services across the Sydney metropolitan areas, we believe in empowering our clients with equity and care by providing accessible and quality aged care services.
Melbar is looking for an energetic and driven Full-time Care Partner to join our supportive and dedicated team.
Our Care Partners play a vital role in assessing, coordinating, and managing the care services for our support at Home clients. As part of a vibrant and dynamic team, the Care Partners ensure our clients receive a holistic approach to clinical and nursing care, with focus on the needs of clients, they provide comprehensive, safe and effective evidence-based care to achieve our clients' individual health outcomes.
Key responsibilities
- Conduct care assessments, develop person-centred care plans and individualised budgets based on participants’ needs, goals and preferences.
- Coordinate and manage services with Service Coordinators, Registered Nurses, allied health professionals and other relevant providers.
- Monitor and review participants’ wellbeing, care needs, service delivery and outcomes, and update care plans as required.
- Monitor individualised budgets, service utilisation and expenditure to ensure appropriate use of available funding.
- Identify, assess and manage risks, incidents, complaints and safeguarding concerns, and escalate clinical issues to relevant professionals.
- Maintain accurate participant records, provide ongoing support and education, and ensure compliance with aged care legislation, quality standards and organisational policies.
Requirements
Essential criteria
- A minimum Certificate III in Individual Support (Ageing), Certificate III in Health Services Assistance, or another relevant Certificate III qualification in aged care, community services or health services.
- Full-time working rights
- Bilingual skills are essential with Mandarin as a compulsory language;
- Valid Australian driver’s license with access to own vehicle
Desirable criteria
- Demonstrated experience in a community/ case manager / nursing role;
- Experience in care management and the Aged Care Quality Standards
- Know Support At Home program
