Clinical Care Supervisor (LPN) - Vancouver, Richmond & North Shore, BC

About the Role

Hero Home Care is seeking an experienced Licensed Practical Nurse (LPN) to join our team as a Clinical Care Supervisor. In this leadership role, you will oversee clinical care delivery, support Community Health Workers, conduct client assessments, and help ensure exceptional care experiences for clients and families across the community.

This position combines clinical expertise, caregiver mentorship, quality assurance, and care coordination in a community and home-care environment. Approximately 50-60% of your time will be spent in the field, with the remainder supporting care planning and clinical administration.

Key Responsibilities

✅ Conduct client assessments and develop care plans

✅ Supervise, coach, and support Community Health Workers

✅ Lead care plan reviews and clinical documentation

✅ Support hospital discharges and client transitions

✅ Monitor quality, safety, and compliance standards

✅ Investigate incidents and support risk management initiatives

✅ Collaborate with families, care teams, and healthcare partners

Qualifications

• Licensed Practical Nurse (LPN) registered and in good standing with BCCNM
• Active CPR/BLS Certification
• 3+ years of Home & Community Care experience
• 2+ years of Hospital or Acute Care experience
• Strong leadership, coaching, and communication skills
• Valid Driver's License and reliable vehicle
• Experience with EMR systems and healthcare technology
