Position: Clinical Support Specialist

The Clinical Support Specialist (CSS) partners with clinical and operational teams to complete a variety of functions that support the Monogram care model and excellent patient experience. The CSS interacts with patients and providers to ensure clinical needs are communicated and met. This individual is a vital asset to the clinical team, and guides patients to meet their preventive, chronic and acute care needs. The CSS reports to the Clinical Support Services Supervisor. This is a remote position requiring the ability to work independently and self-motivate.

Responsibilities

• Oversee and facilitate all Monogram clinician-driven orders including external referrals, prior authorizations, lab orders, prescriptions, durable medical equipment (DME), etc.
• Manage inbound and outbound medical record requests in compliance with HIPAA and organizational policies
• Retrieve, review, and upload medical records from external providers, hospitals, and health systems
• Extract and document pertinent clinical information including lab values, diagnostic reports, medication histories, and specialist notes
• Ensure accurate indexing, categorization, and entry of clinical documentation into patient charts
• Support clinicians by proactively identifying missing documentation or gaps in records
• Participate in special projects including data audits, reporting initiatives, and workflow optimization efforts
• Always maintain strict confidentiality and compliance with HIPAA regulations
• Meet productivity, turnaround time, and quality benchmarks
• Assist clinicians as needed

Position Requirements

• High school diploma or GED required
• Minimum of two (2) years working in the health care industry
• Medical Assistant certification preferred
• Demonstrated customer service and interpersonal skills
• Evidence of time management skills and attention to detail
• Technology proficient with the ability to learn to use a variety of software reporting
• Ability to prioritize, learn, and adapt quickly in a fast-paced environment
