Sample Supports’ is seeking caring and dependable independent contractors to provide individualized residential support to adults with intellectual and developmental disabilities receiving Colorado HCBS waiver services. Contract opportunities may include: Host Home Provider: The individual receiving services lives in the provider’s home and receives individualized support as part of the household. Family Caregiver: A qualified family member provides approved services to an individual who lives in the family home. Sample Supports carefully matches individuals and providers based on compatibility, lifestyle, experience, home environment, location, and caregiving approach. We support individuals with a wide range of needs and welcome both experienced caregivers and people who are new to disability services. Required Application Steps To be considered, applicants must complete both: Culture Index Survey Host Home Provider Questionnaire The Culture Index is not a test, and there are no right or wrong answers. It helps us understand how you work and identify placements where both you and the individual receiving services can thrive. About the Opportunity Host Home Providers and Family Caregivers provide consistent, person-centered support in a home and community setting. Services are based on the individual’s person-centered service plan, assessed needs, preferences, rights, and goals. Support needs may include personal care, medication administration, meal preparation, transportation, household routines, community participation, relationship building, behavioral support, and developing greater independence. This is an independent contractor opportunity, not an employee position. Providers manage their own schedules and household routines while meeting all contractual, regulatory, documentation, training, and service requirements. Provider Responsibilities Provide a safe, stable, respectful, and welcoming home environment Deliver services according to the individual’s person-centered service plan, behavior plan, protocols, and authorized HCBS services Support the individual’s dignity, privacy, rights, choice, independence, and community involvement Assist with activities of daily living based on individual needs, including bathing, dressing, hygiene, mobility, and other personal care Assist with meal planning, grocery shopping, cooking, household routines, and maintaining a clean living environment Support participation in community activities, relationships, employment, day services, appointments, and personal interests Provide safe transportation to appointments, activities, work, programs, and other authorized destinations Administer or support medications as authorized and trained Implement person-centered behavioral strategies and approved behavior support plans Respond appropriately to behavioral, medical, or environmental emergencies Complete accurate and timely electronic documentation for all services provided Maintain required records and participate in monitoring, home visits, meetings, and quality-assurance reviews Communicate regularly with Sample Supports, the individual’s team, and involved family members or guardians Report changes in health, safety, behavior, or support needs promptly Report suspected abuse, neglect, exploitation, mistreatment, or other reportable incidents as required Coordinate planned respite and communicate provider absences in advance Maintain professional boundaries and follow confidentiality requirements Complete and maintain all required training, certifications, background checks, and provider qualifications Follow applicable federal and Colorado HCBS requirements, agency policies, and the independent contractor agreement Support Provided by Sample Supports Orientation and ongoing training related to intellectual and developmental disabilities Person-centered and trauma-informed support practices Individual-specific training before and throughout the placement CPR, First Aid, QMAP, and required safety training Positive behavior support and adaptive behavior planning Crisis prevention, intervention, and emergency-response planning In-home and community-based consultation from experienced clinical and residential team members 24-hour on-call support Proactive respite planning support Access to eligible provider recognition and support opportunities, including the Dream Project Compensation Compensation varies based on the individual’s authorized services, assessed level of support, placement arrangement, and applicable waiver reimbursement. Provider payments generally range from $1,200 to $5,000 per month. Eligible placements may also include room-and-board payments. Certain qualified difficulty-of-care payments may be excluded from federal taxable income under applicable tax rules. Sample Supports does not provide tax or legal advice, and providers are encouraged to consult a qualified tax professional regarding their individual circumstances. Compensation is associated with an active placement and is not guaranteed before a provider and individual are successfully matched. Provider Qualifications Must be at least 18 years old Must be able to pass required criminal background and Child and Adult Protection Services checks Must have a valid driver’s license Must have dependable transportation and meet motor vehicle record requirements Must be able to transport the individual safely when transportation is part of the service plan Must provide an appropriate and approved home environment Must provide the individual with a private bedroom unless another arrangement is specifically authorized Must allow required home inspections, monitoring visits, and quality-assurance reviews Must be able to complete electronic documentation accurately and on time Must complete and maintain required training and certifications Must be willing and able to perform Nonviolent Crisis Intervention techniques when required by the individual’s needs and provider training Must be able to meet the individualized physical demands of the placement, which may include lifting or assisting with transfers Must maintain reliable communication and respond appropriately to urgent situations Must be willing to collaborate with the individual, family or guardian, case management agency, and Sample Supports team Knowledge of intellectual and developmental disabilities, mental health needs, and trauma-informed care is preferred but not required All adult household members must complete required screening and training when applicable Final approval is based on successful completion of all screening, training, home-safety, documentation, and matching requirements. Completing the application process does not guarantee a placement.
