About Camden
Camden is changing on the inside to make life better for everyone. Our residents and communities are at the heart of everything we do. We’re home to the most important conversations happening today and we’re making radical social change a reality, so that nobody gets left behind. Here’s where you can help decide a better future for us all.
How You’ll Be Involved
As a Mental Health Pathway Referrals Co-ordinator, you will play a vital role in helping some of Camden’s most vulnerable residents access the housing and support they need to live independently and thrive within their communities. Working as part of the Pathways Move On Team, you will coordinate referrals into specialist mental health supported housing, working closely with housing, health and support partners to ensure people can move into appropriate accommodation as quickly as possible. Combining operational delivery with partnership working, you will help develop move-on opportunities, prevent homelessness, and support positive outcomes for residents with severe and enduring mental health needs, making a meaningful difference to people’s lives at key moments of change.
This post is exempt from the Rehabilitation of Offenders Act and will require an Basic DBS.

You’ll be confident communicating with people from a wide range of backgrounds, able to manage sensitive situations with empathy, and committed to making services accessible, inclusive and responsive to individual needs. Comfortable using systems and data to support effective decision-making, you will have strong organisational skills and a commitment to continuous improvement.
