Outpatient Quality Improvement Consultant (QIC)

Apply Description Location: Remote, preferably located in Florida, Georgia, Alabama, Tennessee, Kentucky, Mississippi, North Carolina, or South Carolina Health Quality Innovators (HQI) is seeking an experienced Quality Improvement Consultant to support outpatient practices throughout the Southeast. This role combines clinical expertise, relationship building, data analysis, and coaching to help providers improve patient outcomes and quality performance. Working as part of HQI's Outpatient Clinical Practice Quality Improvement team, you will partner with practice leaders, clinical teams, and stakeholders to identify opportunities for improvement, implement evidence-based strategies, and achieve measurable results. What You'll Do Provide consultation and technical assistance to outpatient clinical practices Analyze quality measure and performance data Conduct needs assessments and root cause analyses Develop and monitor quality improvement action plans Facilitate educational programs, coaching calls, and webinars Build relationships with practice leadership and interdisciplinary teams Support implementation of CMS quality initiatives and best practices Collaborate with subject matter experts and quality improvement leaders Why Join HQI? Fully remote work environment Supportive and collaborative culture Opportunity to improve care for thousands of patients across the Southeast Professional development and growth opportunities Meaningful mission-driven work Requirements Deep knowledge of HCC (Hierarchical Condition Category) coding, risk adjustment, chronic condition documentation, and RAF score accuracy Recent experience in an outpatient practice, health system, or value-based care organization Experience with quality improvement or healthcare operations Knowledge of outpatient practice quality measures and performance improvement practices Strong presentation, facilitation, and relationship-building skills Ability to work independently in a remote environment Willingness to conduct occasional onsite visits within assigned states Highly Desired Outpatient practice quality improvement experience Certified Risk Adjustment Coder (CRC) certification Prior QIO/QIN-QIO experience Experience leading practice-level performance improvement initiatives Ideal Candidate: The ideal candidate combines strong quality improvement and data analysis skills with knowledge of HCC coding, risk adjustment, chronic disease management, and performance measurement. They are comfortable translating complex quality and risk-adjustment data into practical improvement strategies, coaching teams through change, and fostering relationships that drive measurable results. Travel: Limited (<15%) state/regional travel may be required for QICs who reside in the Southeast to visit provider locations. Some non-standard hours may be required for QICs who reside outside the Eastern time zone to support providers or collaborate with colleagues. Annual travel for an all-company meeting is required.
