We believe that high-performing teams drive groundbreaking medical discoveries and invite all applicants to join us and experience what it means to be part of Mass General Brigham.

Job Summary

The Mass General Brigham Population Health Services Organization is seeking applicants for the Population Health Fellow position, a one- to two-year fellowship opportunity for primary care and specialty providers who will complete residency training in July 2027 and are interested in advancing their careers in population health, value-based care, health system transformation, and clinical leadership.

General Summary

The Population Health Fellow will join a multidisciplinary team focused on improving outcomes for patients across Mass General Brigham through value-based care strategies, care model innovation, performance improvement, health equity initiatives, and operational excellence. The Fellow will work closely with population health clinical and operational leaders to understand how programs are designed, implemented, measured, and scaled across a large integrated health system.

The fellowship is designed for board-eligible graduating residents who want to develop the skills needed to lead population health programs and value-based care initiatives. Fellows will spend approximately 30% of their time in clinical practice, with the remaining time dedicated to didactic training, mentorship, project development, program implementation, data analysis, and collaboration with population health medical directors and operational teams.

Principal Duties and Responsibilities

• Collaborate with interdisciplinary clinical, operational, analytics, quality, and strategy teams to design, implement, and evaluate population health initiatives.
• Develop and lead one or more population health projects aligned with Mass General Brigham priorities, including value-based care, care management, primary care transformation, specialty care integration, quality improvement, health equity, or patient experience.
• Use data, dashboards, literature review, stakeholder interviews, and program evaluation methods to identify opportunities for improvement and make evidence-informed recommendations.
• Partner with population health medical directors and operational leaders to understand Accountable Care Organization strategy, payer contract performance, quality metrics, care management programs, risk capture, and financial and operational considerations.
• Translate clinical experience into practical recommendations for care model design, workflow improvement, and patient-centered interventions.
• Participate in educational activities, mentorship meetings, leadership development opportunities, and didactic training related to population health and value-based care.
• Prepare polished deliverables such as project charters, workplans, implementation plans, data summaries, presentations, and written recommendations for leadership review.
• Engage with stakeholders across primary care, specialty care, community health, care continuum, behavioral health, pharmacy, analytics, and Mass General Brigham Health Plan as appropriate to assigned projects.
• Maintain a clinical practice commitment that supports ongoing connection to frontline care delivery and informs population health work.

Program Structure

The fellowship begins in July 2027 and is structured as a one-year program with the possibility of a second year based on program needs, fellow goals, and funding. Fellows are expected to maintain approximately 30% clinical practice time and spend approximately 70% of their time with the Population Health Services Organization. The PHSO team works in a hybrid model, with onsite presence at Mass General Brigham corporate headquarters and other clinical or operational sites as needed.

Education and Didactics

The Population Health Fellow will participate in a structured education and didactic curriculum designed to build foundational knowledge and applied skills in population health, value-based care, clinical operations, and health system leadership. Didactic time will complement project-based learning and clinical practice, allowing the Fellow to connect concepts from the classroom, leadership meetings, data review, and frontline care delivery.

• Population health strategy, including accountable care, risk contracts, quality performance, care management, and total cost of care.

• Primary care and specialty care transformation, including team-based care, transitions of care, chronic disease management, referral optimization, and care model redesign.

• Health equity and community health, including approaches to identifying disparities, designing equitable interventions, and measuring impact.

• Data and analytics for population health, including dashboard interpretation, metric selection, program evaluation, and communicating results to clinical and operational audiences.

• Quality improvement and implementation science, including problem definition, stakeholder engagement, change management, high reliability organization strategies and sustainability planning.

• Leadership and professional development, including communication, meeting facilitation, executive presence, project management, and influencing without formal authority.

• Health policy, payment models, and health system finance, including how clinical, operational, and financial priorities intersect in value-based care.

Educational activities may include seminars, case discussions, leadership meetings, project workshops, mentorship sessions, and opportunities to present work to Population Health leaders and partner teams. The Fellow will receive regular feedback and coaching from clinical and operational mentors and will be encouraged to develop a learning plan that aligns fellowship experiences with long-term career goals.
