Working alongside a wider clinical team, you'll deliver high-quality care while managing a defined caseload within your agreed competencies x1 Permanent Full Time role & x1 Fixed Term (12 months) 26 hours per week role available. What You'll Do:
- Support patient assessments and deliver individual care and rehabilitation programmes
- Help people take part in healthy living, wellbeing and therapy activities
- Monitor progress, recognise changes in a person's health and raise concerns appropriately
- Work closely with patients, carers, colleagues and other services to provide coordinated care
- Support and supervise less experienced colleagues with agreed clinical tasks
- A relevant Level 4 qualification, Foundation Degree in Health and Social Care, or demonstrable rehabilitation experience
- Experience within a clinical care setting and promoting healthy living
- Good communication, organisation and teamwork skills, with confidence using initiative appropriately
- An understanding of rehabilitation, including safe exercise and when to escalate clinical concerns
