Job Summary

Are you an organised, proactive individual with excellent administrative and coordination skills? We have an exciting opportunity to join our Urgent & Emergency Care team within Solihull Community Services on a 12-month fixed-term contract.

Working as part of a dedicated multidisciplinary team, you will support our Care Coordination and In-Reach Nursing teams to help deliver high-quality care closer to home. This is a rewarding role where you will play a vital part in ensuring patients receive timely, coordinated care within the community, helping to avoid unnecessary hospital admissions and supporting safe, effective discharge from hospital.

Main Duties, Tasks & Skills Required

As a key member of the team, you will provide comprehensive administrative and coordination support, working closely with clinical colleagues, operational teams, patients, carers, and partner organisations. You will track patients throughout their care journey, ensuring referrals, admissions, discharges, transfers, and ongoing care plans are accurately managed and recorded.

You will contribute to the smooth flow of patients across urgent and emergency care pathways by maintaining accurate patient records, supporting waiting time standards, attending daily handovers and multidisciplinary meetings, and sharing key information to enable informed clinical and operational decision-making.

This role requires excellent organisational skills, attention to detail, and the ability to use initiative while managing a varied workload. You will be a professional, compassionate, and approachable point of contact for patients and service users, maintaining confidentiality and delivering a high standard of customer service at all times.

Qualifications

Essential

  • Knowledge of administrative and operational procedures, including the use of specialist IT systems, information management, and basic project or process coordination, acquired through formal training and/or relevant experience to diplomalevel equivalent.

Experience

Essential

  • Experience of providing administrative or operational support in a patientfacing or patientrelated role within an NHS provider organisation.
  • Experience of working collaboratively within a multidisciplinary team in an acute healthcare setting.
  • Experience of acting as a point of contact for enquiries from internal and external stakeholders, including patients and families.
  • Ability to apply judgement and assess situations where there may be more than one appropriate course of action.
  • Experience of managing routine and nonroutine workloads, including responding to unpredictable and competing demands.
  • Understanding of the scope of the role within the team and organisation, and how the role contributes to effective service delivery and service development.
  • Knowledge of administrative procedures and use of NHS information systems containing patient data (e.g. PAS or equivalent).
  • Knowledge of clinical terminology and patient flow processes within an acute hospital environment.
  • Knowledge of data protection and confidentiality requirements relevant to the role.

Desirable

  • Experience of working within patient flow, bed management, admissions, or discharge coordination processes.

Additional Criteria

Essential

  • Ability to work on own initiative within agreed procedures.
  • Ability to travel between sites as required to support the needs of the service.
