Job Summary
An excellent opportunity has arisen for an experienced Physiotherapist looking to develop specialist skills in frailty, acute care and admission avoidance within a supportive and innovative multidisciplinary team. The Older Persons Assessment and Liaison (OPAL) Team at Good Hope Hospital are looking for an individual with a genuine passion for working with older people and delivering high-quality, patient-centred care.
The service supports admission avoidance, prevents deconditioning, and promotes independence through specialist assessment and treatment within the Emergency Department, Acute Medical Unit, and Medical Assessment Unit.
This is an exciting time to join the service as OPAL continues to expand, with the development of the Frailty Same Day Emergency Care (FSDEC) unit to provide timely coordinated care for those with urgent healthcare needs that cannot be managed within the community, but who do not require an overnight hospital admission.
A Home First model, OPAL is committed to supporting people to remain independent and return to their home wherever possible. Close partnership working with community services and wider healthcare providers, ensures patients receive the right care, in the right place, at the right time.
OPAL is a seven-day service and works collaboratively across disciplines to ensure patients receive timely physiotherapy input when it is needed most.
Main Duties, Tasks & Skills Required
As a Band 6 Physiotherapist, you will use your clinical expertise, leadership skills and knowledge of frailty to assess, treat and support patients throughout their journey, enhancing both patient outcomes and experience. You will be expected to work autonomously, contribute to service development, support junior staff and students, and play an active role within the multidisciplinary team.
The role requires excellent communication and interpersonal skills, with the ability to manage sensitive and complex conversations with patients, relatives and carers. You will also be confident in coordinating onward referrals and signposting patients to appropriate community services and support pathways to facilitate safe and effective discharge planning.

- Diploma / degree in Physiotherapy.
- Registered with Healthcare Professions Council (HCPC).
- A levels or equivalent qualification.
- Good standard of general education including GCSE Maths and English grade C and above.
- Evidence of CPD including recent post graduate training relevant to clinical speciality.
- Member of specialist interest group.
- Attendance at Clinical Educators course.

- Demonstrates clinical experience enabling the post holder to manage a caseload of patients within this speciality / within the NHS with experience of elderly and frail elderly, respiratory, neurological and musculoskeletal conditions.
- Experience of working with student physiotherapists and assistants.
