Overview

• Specialty: Family Medicine / Internal Medicine
  
• Location: Bolton Area, Connecticut
  
• Practice Setting: Office-based outpatient Primary Care
  
• Work Schedule: Full-time weekdays; four ten-hour days are offered.
  
• Support Staff / Resources: Dedicated Medical Assistant and clinical support; Behavioral Health, high-risk nurse care management, Social Work, and specialty referrals
  
• EMR: Epic with Dragon and Ambient AI; electronic patient communication tools

What You Can Expect

• Provide prevention, acute assessment, and longitudinal chronic disease management.
  
• Coordinate specialty referrals and community-based follow-up.
  
• The practice serves patients from birth through adulthood.

Requirements

• MD or DO; Board Eligible or Board Certified in Internal Medicine or Family Medicine.
