Seeking a licensed Physical Therapist to deliver personalized, in-home and senior living care, including virtual options, to older adults.

Key Responsibilities

Provide one-on-one therapy in various settings, including private homes and senior communities.
Conduct evaluations, develop individualized treatment plans, and administer evidence-based therapy to improve mobility, independence, and wellness.
Document patient progress accurately in EMR systems and collaborate with an interdisciplinary team.
Educate patients, caregivers, and staff on safety and functional aging strategies.
Support virtual care delivery when appropriate.

Qualifications & Skills

Valid PT license, degree from an accredited program, and CPR certification.
Proficiency with basic computer and EMR systems.
Ability to meet physical demands, including lifting, repositioning, and extended walking.
Willingness to drive and wear PPE as needed.
