Role: Physical Therapist providing personalized in-home, senior living, and virtual care to older adults.
Responsibilities: Conduct evaluations, develop treatment plans, and deliver evidence-based therapy sessions to support mobility, independence, and wellness. Document care in EMR, collaborate with an interdisciplinary team, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications & Skills: Valid PT license or eligibility, degree from an accredited program, basic computer/EMR skills, and current CPR certification. Physical ability to perform transfers, reposition patients, and drive to care locations. Must meet physical demands of in-home and community-based care.
