Role: Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults.
Responsibilities: Conduct evaluations, develop personalized treatment plans, deliver evidence-based therapy to enhance mobility, independence, and wellness. Document patient progress in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications & Skills: Valid PT license, degree from an accredited program, proficiency with EMR and computers, current CPR certification. Physical ability to perform transfers, reposition patients, and drive to patient locations. Capable of exerting physical effort and wearing PPE.
Key Duties: Provide one-on-one therapy in various settings, support aging-in-place, promote safety, and support patient longevity. Flexibility to create treatment schedules and leverage virtual care options.
