Physical Therapist
Provides outpatient rehabilitation services primarily in-home for older adults, evaluating and treating patients in their residences or independent living facilities. Responsibilities include developing personalized therapy programs to improve mobility, strength, balance, and functional performance, along with educating patients and caregivers to promote independence. The role involves supervising support personnel, adhering to treatment schedules, documenting clinical activities, and collaborating with the rehabilitation team to optimize patient care. Therapists perform assessments, design treatment plans, and utilize techniques such as therapeutic exercises, manual therapy, modalities, and adaptive device fitting. They also conduct fall risk assessments, provide case management, and ensure a safe environment. Qualifications include a valid Physical Therapy license, a Master’s degree in Physical Therapy or equivalent, and reliable transportation.
