Summary
We are seeking a Physical Therapist to provide outpatient rehabilitation services primarily in in-home settings for older adults, including private residences and independent living facilities. The role involves assessing patient needs, developing and implementing individualized treatment plans, and educating patients and caregivers to promote independence and health. Responsibilities include supervising support personnel, ensuring documentation compliance, collaborating with the rehabilitation team, and making referrals to other disciplines as needed. The therapist will utilize various therapeutic techniques such as therapeutic exercises, manual therapy, modalities, and adaptive device training. Additionally, they will perform evaluations, modify treatment plans, and conduct home and community mobility assessments. Qualifications include a valid state license, relevant degrees, and reliable transportation.
