We are seeking a dedicated Physical Therapist to provide outpatient rehabilitation services in in-home and facility settings for older adults.

- Assessing, developing, and implementing individualized therapy programs to restore and enhance patient function.
- Educating patients and caregivers to promote independence and safety.
- Referring patients to other disciplines as needed and collaborating with a rehabilitation team.
- Supervising support personnel and ensuring timely documentation and adherence to treatment plans.
- Performing evaluations, functional assessments, and using therapeutic techniques such as therapeutic exercises, manual therapy, and modalities.
- Designing and fitting adaptive devices, providing mobility training, and performing home safety assessments.
- Valid licensure in the state(s) of practice or eligibility.
- Master’s degree in Physical Therapy or equivalent, with experience or additional degrees as applicable.
- Reliable transportation and a valid driver’s license.
