Seeking a full-time Physical Therapist for outpatient rehabilitation services in various home-based settings, primarily serving the older adult population.

Responsibilities Include

Assessing, developing, and delivering individualized therapy programs to promote recovery, movement, and independence.
Evaluating patient needs, making referrals to other disciplines, and modifying treatment plans accordingly.
Instructing patients and caregivers on skills to enhance function and safety.
Supervising support personnel and ensuring adherence to schedules, documentation, and safety standards.
Performing exams, designing therapeutic interventions, and utilizing techniques such as therapeutic exercises, manual therapy, modalities, and adaptive device application.
Conducting home and community mobility assessments and promoting health and wellness initiatives.

Qualifications Include

Valid Physical Therapy license in the practicing state(s) or license eligibility.
Master’s degree in Physical Therapy or equivalent, or a Bachelor’s degree plus 5 years of experience.
Evaluation of foreign degrees for U.S. equivalency, if applicable.
Reliable transportation and a valid driver’s license.
