Physical Therapist (Sign On Bonus)

We are seeking a licensed Physical Therapist to provide personalized, in-home, senior living, and virtual care to older adults in Hyattsville, MD.

Key Responsibilities Include

• Deliver one-on-one physical therapy sessions in various settings, including private residences and senior communities
• Perform evaluations, develop tailored care plans, and implement evidence-based treatments to enhance mobility, independence, and quality of life
• Document patient progress accurately within electronic medical records
• Collaborate with an interdisciplinary team of healthcare professionals
• Educate patients, caregivers, and staff on safety, function, and aging-in-place strategies
• Utilize telehealth and virtual care to increase access and flexibility
  
  

Qualifications Include

• Valid Physical Therapy license and degree from an accredited program
• Proficiency in basic computer and EMR systems
• Current CPR certification
  
  

Physical demands: Ability to exert moderate effort, assist with patient transfers, perform therapeutic activities, and drive to patient locations while maintaining safety protocols.
