Registered Nurse – Heart Failure Clinic

Role Overview: The Care Team RN provides comprehensive nursing care within the Cardiology Outpatient Practice, primarily in the Heart Failure Clinic, along with coverage for Amyloid and Sarcoid Clinics.

Key Responsibilities

• Perform face-to-face and non-face-to-face patient interactions, including pre-visit preparation, direct care, and patient/family education.
• Coordinate transitions of care and collaborate effectively with providers and team members.
• Support professional nursing practice across various settings, adhering to the Mayo Nursing Care Model and ANA standards.
• Lead and mentor through roles such as preceptor, participate in quality improvement, and delegate care based on skill, experience, and patient needs.
  
  

Qualifications & Skills

• Graduate of an accredited nursing program with at least one year of RN experience, preferably with cardiac care experience.
• Excellent verbal and written communication, teamwork, and critical thinking skills.
• Proficiency in computer use and electronic medical records preferred.
• Current RN licensure, BLS certification, and ability to work flexible hours are required.
